Hello 40s

For the decade when several things start moving at once

Designed for women entering their 40s and the perimenopausal transition, this panel assesses the hormonal markers relevant to this life stage alongside thyroid function, bone health, cardiovascular risk and metabolic health.

For those considering pregnancy or already on a fertility journey, it also includes a marker of ovarian reserve to support an informed family planning discussion.

The purpose is to help you understand the changes taking place, and to support informed decisions about managing symptoms and protecting long-term wellbeing.

Hormone levels in the years before menopause fluctuate substantially from week to week. A single FSH and oestradiol reading is a snapshot, not a verdict, and perimenopause is diagnosed primarily from symptoms and history rather than from a blood test. We include these markers because they are useful in context and help exclude other causes. We will not tell you a number has diagnosed your menopause.

What this panel assesses

  • Hormonal: FSH, Oestradiol, Testosterone, SHBG, Prolactin

  • Ovarian reserve: AMH (see note below)

  • Thyroid: TSH, Free T3, Free T4, Thyroid peroxidase antibodies

  • Cardiovascular and metabolic: Full lipid profile, HbA1c, Fasting insulin, hsCRP

  • Bone: Calcium, Phosphate, Alkaline phosphatase, Vitamin D

  • General and nutrition: Full blood count, Ferritin and iron studies, Liver function, Kidney function · Vitamin B12, Folate

What happens next

  • Where results point towards perimenopause, you will have a proper consultation about options including HRT, and including the option of doing nothing.

  • Where cardiovascular or metabolic markers are drifting, you will see our dietitian.

  • Where bone markers or vitamin D are low, we will address it and discuss whether a DEXA scan is worth arranging
